Differential Diagnosis Assessment: Complete a Differential Diagnosis Assessment using the template forms uploaded into Canvas. There is one form for persons aged 0-17 and another form for persons aged 18 +. Assessments should be completed on persons served at the students’ internship placement. Students not currently in a field placement or those placed in Macro placements may choose from the list off movies provided by the instructor. All names shall be changed or only initials used to protect confidentiality. The rubric below indicates the areas on which you are being evaluated for each of the assessments. The assessment should be free from acronyms, spelling and grammar errors. Writing should be clear and concise. The assessment shall show insight regarding the person’s struggles, as well as clinical and critical thinking. Professional and clinical terminology should be used.
